Provide a link between patients and available resources to provide the necessary resources in a cost-effective manner.

• Provide home visits to risk patients and families for evaluation of needs in an effort to provide direction of care for desired outcomes in a cost-effective manner.
• Report specific health & social information back to the care management team to asset in development of care pathways.
• Participate in multidisciplinary team conferences as needed.
• Assist patients with accessing needed services, recognize potentially serious problems to prevent poor health and social outcomes.

• High school diploma or equivalent.
• Minimum of 1 year of experience working with a community agency providing health care services, including community resources, communication skills, individual and family advocacy, health education, and service skills.
• Active patient of the state's Community Health Worker Association.
• Effective communication skills.
• Extensive experience with Microsoft Office Suite (Word, PowerPoint, Excel).
• Professional work ethic.
• Strong comfort with working in a fast-paced environment.
• An authentic passion for improving the lives of underserved patients and advancing value-based care.

• Community Health Worker (CHW) certification through a college-level program.

• Ohio Plan: CHW certification through a college-level program or LPN or LCSW license required. Valid driver's license required.
• Michigan Plan: CHW certification preferred but not required. Valid driver's license required.
• Illinois Plan: CHW certification preferred but not required. Valid driver's license required.
• Medicare: CHW certification through a college-level program or LPN license required.
• Valid driver's license required.
• Kentucky Plan: CHW certification preferred but not required. Valid driver's license required.

• Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 19500 HWY 249, Suite 570 Houston, TX 77070.
• The total compensation target pay range for this role is: $23 - $28 per hour. The range represents our national target range for this role.
